Vision Exam Requirement
All children enrolling in kindergarten or for the first time in an Illinois school shall have an eye examination. This eye examination shall be performed by a licensed optometrist or ophthalmologist. The eye exam must be completed within the calendar year prior to the start of school and be submitted to your child's school health office prior to October 15.
