Completed physical forms may be turned in to the appropriate school health office as soon as they are completed. Students not in compliance with the physical and immunization requirement will be excluded from school on October 15. Remember that all incoming kindergarteners need to have a physical exam and all booster shots, in addition to a dental exam and an eye-exam done by an eye doctor. Children entering second grade need a dental exam and children entering 6th grade need a physical exam and a dental exam. Some of you may be experiencing some hardship in getting health care for your children secondary to cutbacks in hours or layoffs at work, resulting in loss of health insurance. If you find that you have no insurance for your children, you may be eligible for the Illinois state health care program for children called All Kids. I have brochures and applications in my office, or you can find out more information by visiting the ALL Kids website, or clicking on the All Kids link on the Health Office web page. The DuPage County Health Department Smile Squad dental van will come to Franklin School in the spring of 2010. More information will be available at a later date. |
