Parents of Incoming Sixth Grader Fall '09
All students entering 6th grade next fall will require a physical examination. During the summer, physical exam forms may be mailed to: Hadley Health Office, 240 Hawthorne Blvd., Glen Ellyn, 60137. If the physical exam is not mailed in over the summer, bring it to Start Up Day on August 17th.
The Dental Examination is required for students entering 6th grade. The examination must be completed by a licensed dentist. Dental exams are valid from Nov. 15, 2008-May 15, 2010.
